If you already have an account, simply click "Login" and enter your credentials.

Fill Out the Form:
Once you are logged in and have completed the payment, you will gain access to the nomination forms, located in the menu. Choose the appropriate nomination form in the drop down. 

Carefully fill out all required fields, providing detailed information about the nominee. This includes their name, contact or business information, category of nomination, and a compelling description of why they deserve to be recognised. 

You can nominate yourself or your business.

Complete the Form:
Ensure that all sections of the form are fully completed. Double-check for accuracy and completeness.

Do not exit the page before submitting, as your work will not be saved and you will need to start over. Once you have reviewed all the details, click the "Submit" button to finalise your nomination.

Nominator Details:

You are required to fill out the "Nominator Details" section. This step is crucial as it  verifies the nominee’s legitimacy. Begin by specifying whether you are nominating yourself or someone else. 

Next, provide your first and last names, contact information including phone number and email address, and your relationship to the nominee. Ensure that your email address is correctly typed to avoid any communication issues. 

Make sure all information is accurate and up-to-date before moving to the next section.

Nominee Details:

You are required to provide comprehensive details about the nominee. This includes the nominee’s full name, contact information such as phone number and email address, and a brief biography. 

Additionally, you should include the nominee’s online presence by providing URLs for their website and social media profiles like Facebook, Instagram, and LinkedIn. 

If nominating a business, include its legal name, Australian Business Number (ABN), and physical address, covering street address, suburb, postcode, and state. These details are crucial as they establish the identity of the nominee and ensure that the competition organisers can contact and verify the successful nominee.

Provide detailed information about why the nominee deserves to be recognised. 

This includes specific achievements, contributions to the pole dancing community, and any other relevant information that supports the nomination. 

Ensure that all sections of the form are fully completed. Do not exit the page before submitting, as your work will not be saved, and you will need to start over. 

Once you have reviewed all the details, click the "Submit" button to finalise your nomination.

Additional Attachments to support your nomination:

Photos can be provided for the presentation evening.

Photos that are offensive will not be accepted.
